Tigecycline

Tigecycline is a modified tetracycline. It works by binding to the 30s subunit of the bacterial ribosome and inhibiting protein synthesis.

Tigecycline is indicated for the following infections-

  • Skin infections caused by susceptible organisms.
  • Intra-abdominal infections.
  • Community acquired pneumonia caused by Hemophilus influenza and Legionella spp.
  • For adults, a starting dose of 100 mg is advised, followed by 50 mg every 12 hours for 5 to 14 days. The length of treatment should be determined by the severity of the illness, its location, and the patient's clinical response.
  • Tigecycline should only be used to treat patients aged 8 and up after consulting with a physician with relevant experience in infectious illness care. Children aged 8 to <12 years: 1.2 mg/kg of tigecycline every 12 hours intravenously to a maximum dose of 50 mg every 12 hours for 5 to 14 days.
  • Adolescents aged 12 to <18 years: 50 mg of tigecycline every 12 hours for 5 to 14 days.
  • The dose of tigecycline should be reduced by half in individuals (including children) with severe hepatic impairment. Following the 100 mg loading dosage, the adult dose should be lowered to 25 mg every 12 hours. Patients with severe hepatic impairment should be handled with caution and their treatment responses should be closely monitored.

The side effects associated with tigecycline is usually mild. They include-

  • Nausea
  • Vomiting
  • Diarrhea
  • Phlebitis
  • Dizziness
  • Dyspepsia
  • Tigecycline should be used with caution in patients with hepatic problems.
  • Pancreatitis have also been reported with the use of tigecycline.
  • Superinfection by resistant organisms may occur.
  • Anaphylaxis/anaphylactoid reactions, potentially life-threatening, have been reported with tigecycline

Contraindication

Tigecycline is contraindicated in patients with known hypersensitivity to tigecycline or any other tetracyclines such as-

Tigecycline should not be taken with milk, antacids & mineral supplements.

Tigecycline is contraindicated in the following conditions-

  • Pregnancy
  • Lactation
