Zoledronic Acid

Zoledronic acid belongs to a class of drugs called the bisphosphonates. The mechanism of action of Zoledronic acid acid is thought to involve the inhibition of osteoclast activity, which are the cells responsible for breaking down bone. By inhibiting the activity of osteoclasts, Zoledronic acid acid slows down the breakdown of bone and increases bone density.

 

 

Zolendronic acid is indicated in-

  • Treatment and prevention of osteoporosis in postmenopausal women
  • Treatment of osteoporosis in men
  • Bone metastases
  • Hypercalcemia of malignancy

 

 

Osteoporosis

  • The usual dose is 5 mg given as one infusion per year into a vein by your doctor or nurse. The infusion will take at least 15 minutes.
  • In case you recently broke your hip, it is recommended that Zoledronic acid 5 mg is administered two or more weeks after your hip repair surgery.
  • It is important to take calcium and vitamin D supplements (for example tablets) as directed by your doctor.
  • For osteoporosis, Zoledronic acid 5 mg works for one year. Your doctor will let you know when to return for your next dose.

Hypercalcemia of malignancy

  • 4 mg as a single-use intravenous infusion over no less than 15 minutes. 
  • 4 mg as retreatment after a minimum of 7 days.

Multiple myeloma and bone metastasis from solid tumors

  • 4 mg as a single-use intravenous infusion over no less than 15 minutes every 3-4 weeks for patients with creatinine clearance of greater than 60 mL/min. 
  • Reduce the dose for patients with renal impairment. 
  • Co-administer oral calcium supplements of 500 mg and a multiple vitamin containing 400 international units of Vitamin D daily.

Administer through a separate vented infusion line and do not allow to come in contact with any calcium or divalent cation-containing solutions.

  • Common side effects include headache, abdominal pain, muscle and joint pain, and flu-like symptoms.
  • Serious side effects include jaw osteonecrosis, allergic reactions, and esophageal problems.

 

 

  • Zoledronic acid may cause a serious condition called osteonecrosis of the jaw. It is recommended to have a dental exam before starting treatment and to practice good oral hygiene.
  • Zoledronic acid may cause serious allergic reactions. Symptoms may include hives, rash, itching, and difficulty breathing.
  • Zoledronic acid may cause serious or life-threatening problems with the esophagus. Symptoms may include difficulty or pain with swallowing, chest pain, or new or worsening heartburn.

 

 

Contraindication

Contraindicated in patients hypersensitive to Zoledronic acid.

 

 

Contraindicated in uncontrolled hypercalcemia (high levels of calcium in the blood).
