Cilastatin + Imipenem

Imipenem is a beta lactam antibiotic of the Carbapenem class. Imipenem is effective against various types of gram positive & gram negative bacteria including the notorious Pseudomonas aeruginosa. It is also effective against certain anaerobes. Imipenem stops the synthesis of bacterial cell wall resulting in the death of the bacteria as they attempt to divide.

Equivalent amount of Cilastatin is combined with imipenem to reduce the nephrotoxic effects of the drug.

Imipenem is used for the treatment of the following cases-

  • Lower Respiratory Tract Infections
  • Urinary Tract Infections (complicated and uncomplicated)
  • Intra-Abdominal Infections
  • Gynecologic Infections
  • Bacterial Septicemia
  • Bone and Joint Infections
  • Skin and Skin Structure Infections
  • Endocarditis

Imipenem is administered as intravenous infusion. The dosage must be administered according to the physician's advice.

Imipenem + cilastatin has the following side effects-

  • Nausea
  • Anaphylaxis & serious hypersensitivity reactions
  • Seizures
  • Clostridium difficile Associated Diarrhea
  • Nephrotoxicity

 

Patients who have previously shown hypersensitivity reactions to other beta lactams like penicillins or cephalosporins may be hypersensitive to Imipenem+Cilastatin. Fatal hypersensitive reaction (Anaphylaxis) have also been reported.

Contraindication

Imipenem+Cilastatin is contraindicated in patients who have shown hypersensitivity to any component of this product.

There is no known contraindications of imipenem+cilastatin in terms of food & drinks

Imipenem can induce seizures. It can interact with valporic acid, an anti convulsant, & reduce its concentration to subtherapeutic levels and induce seizures.
Although nephrotoxic effects of Imipenem is reduced by co-administering cilastatin, dosage adjustment may still be required for patients with compromised renal funtion.
