Rosiglitazone

Rosiglitazone is a hypoglycemic drug from the class thiazolidinediones. Rosiglitazone binds & activates to a nuclear receptor, called the PPAR-γ. PPAR-γ then binds to the DNA and retinoid X receptor at the same time and increases the transcription of insulin sensitive genes. The end result is that the peripheral tissues become sensitized to insulin.

Rosiglitazone is indicated in the management of type-2 Diabetes Mellitus.

  • Rosiglitazone can be given in a single daily dose of 4 mg or in two divided doses of 4 mg.
  • Patients who do not respond adequately after 8 to 12 weeks of treatment, as measured by a decrease in fasting plasma glucose (FPG), may have their dose increased to 8 mg daily.
  • Increases in Rosiglitazone dose should be accompanied by careful monitoring for fluid retention-related adverse events. You can take Rosiglitazone with or without food.
  • Rosiglitazone should not be taken in excess of 8 mg per day.

Sid effects reported with the use of drugs include-

  • upper respiratory tract infection
  • headache
  • weight gain
  • atherosclerosis
  • hypoglycemia
  • Rosiglitazone, like other thiazolidinediones, can cause fluid retention, which can exacerbate or lead to heart failure when taken alone or in combination with other diabetes medications. Heart failure symptoms and signs should be monitored in patients. If these signs and symptoms appear, the heart failure should be managed in accordance with current best practices. Furthermore, rosiglitazone discontinuation or dose reduction must be considered.
  • Rosiglitazone increased the risk of congestive heart failure in trials where it was combined with insulin. It is not recommended to take Rosiglitazone with insulin at the same time.
  • In patients with edema, Rosiglitazone should be used with caution.

Contraindication

  • Contraindicated is patients hypersensitive to Rosiglitazone or similar drug-

 

 

Contraindicated in patients with Heart Failure.
