Cilostazol

Cilostazol is a Platelet aggregation inhibitor and a vasodilator. Cilostazol inhibits phosphodiesterase III, an enzyme responsible for the breakdown of cyclic adenosine monophosphate (cAMP) in platelets and vascular smooth muscle cells. By inhibiting this enzyme, cilostazol increases intracellular cAMP levels, leading to inhibition of platelet aggregation and vasodilation.

 

  • Cilostazol is indicated for the improvement of symptoms of intermittent claudication, a condition characterized by pain, cramping, or weakness in the legs during exercise due to reduced blood flow.
  • It is used to increase walking distance and delay the need for invasive procedures such as bypass surgery or angioplasty.

 

  • The recommended dosage of cilostazol is 100 mg orally twice daily, at least 30 minutes before or 2 hours after meals.
  • Treatment duration may vary depending on the patient's response and the treating physician's discretion.
  • Cilostazol should be used in conjunction with other interventions such as exercise, weight control, and management of risk factors.

The following side effects have been reported with the use of cilostazole-

  • Headache
  • Diarrhea
  • Abdominal pain
  • Palpitations
  • Dizziness
  • Nausea
  • Vomiting
  • Rash
  • Increased heart rate
  • Edema

 

  • Cilostazol may increase the risk of bleeding, particularly in patients with bleeding disorders or those taking anticoagulant medications. Close monitoring for signs of bleeding is recommended.
  • Patients with heart failure should be closely monitored during cilostazol therapy as the drug may exacerbate symptoms.
  • Cilostazol is metabolized by the liver, so caution is advised in patients with severe liver impairment.
  • The safety and efficacy of cilostazol in pediatric patients have not been established, and its use in this population is not recommended.
  • Cilostazol should be used with caution in patients with a history of bleeding disorders, congestive heart failure, or low blood pressure.
  • Cilostazol is contraindicated in patients with heart failure of any severity. Cilostazol and several of its metabolites are inhibitors of phosphodiesterase III. Several drugs with the pharmacologic effect have caused decreased survival compared to placebo patients with class III-IV heart failure. 

 

Contraindication

Cilostazol contraindicated in patients with known hypersensitivity to cilostazol or any of its components.

 

Cilostazol is contraindicated in patients with heart failure of any severity.
