Esomeprazole

Esomeprazole is a selective and irreversible proton pump inhibitor. It suppresses stomach acid secretion by specific inhibition of the H+/K+-ATPase system found at the secretory surface of gastric parietal cells.

Esomeprazole is indicated for the following conditions-

  • Healing of Erosive Esophagitis
  • Maintenance of Healing of Erosive Esophagitis
  • Symptomatic Gastroesophageal Reflux Disease
  • Risk Reduction of NSAID-Associated Gastric Ulcer
  • H. pylori Eradication to Reduce the Risk of Duodenal Ulcer Recurrence
  • Pathological Hypersecretory Conditions Including Zollinger-Ellison Syndrome

 

Gastroesophageal Reflux Disease (GERD):

  • Healing of Erosive Esophagitis: 20 mg or 40 mg Once Daily for 4 to 8 Weeks
  • Maintenance of Healing of Erosive Esophagitis: 20 mg Once Daily
  • Symptomatic Gastroesophageal Reflux Disease: 20 mg Once Daily for 4 Weeks

Pediatric GERD:

  • 12 to 17 Year Olds:
    • Healing of Erosive Esophagitis: 20 mg or 40 mg; Once Daily for 4 to 8 Weeks.
    • Symptomatic GERD: 20 mg Once Daily for 4 Weeks.
  • 1 to 11 Year Olds:
    • Short-term Treatment of Symptomatic GERD: 10 mg Once Daily for up to 8 Weeks

Healing of Erosive Esophagitis:

  • weight < 20 kg: 10 mg Once Daily for 8 Weeks
  • weight > 20 kg: 10 mg or 20 mg Once Daily for 8 Weeks

1 month to < 1 year:

  • Erosive esophagitis due to acid-mediated GERD
    • weight 3 kg to 5 kg 2.5 mg Once Daily for up to 6 Weeks
    • weight > 5 kg to 7.5 kg 5 mg Once Daily for up to 6 Weeks
    • weight >7.5 kg to 12 kg 10 mg Once Daily for up to 6 Weeks

Risk Reduction of NSAID Associated Gastric Ulcer :20 mg or 40 mg Once Daily for up to 6 months

  • H. pylori  Eradication to Reduce the Risk of Duodenal Ulcer Recurrence:
    Triple Therapy:
    • Esomeprazole-40 mg Once Daily for 10 Days
    • Amoxicillin 1000 mg Twice Daily for 10 Days
    • Clarithromycin 500 mg Twice Daily
  • Pathological Hypersecretory Conditions Including Zollinger-Ellison Syndrome:  40 mg Twice Daily for 10 Days

Commonly reported side effects include-

  • Headache
  • Nausea
  • Abdominal pain
  • Diarrhea
  • Vomiting
  • Flatulence
  • Upper respiratory tract infection (pharyngitis, sinusitis)
  • Dizziness
  • Asthenia
  • Cough
  • A symptomatic response to Esomeprazole medication does not rule out the possibility of stomach cancer.
  • Gastric corpus biopsies from individuals treated long-term with omeprazole, of which esomeprazole is an enantiomer, have occasionally revealed atrophic gastritis.
  • Patients taking PPIs, such as Esomeprazole, have developed acute interstitial nephritis.
  • Long-term usage of any acid-suppressing medicine (e.g., more than 3 years) might result in cyanocobalamin (vitamin B-12) malabsorption due to hypo- or achlorhydria.
  • PPIs, such as Esomeprazole, have been linked to an increased risk of Clostridium difficile-related diarrhea, particularly in hospitalized patients.
  • Avoid taking Esomeprazole and clopidogrel at the same time. Clopidogrel is a prodrug that is used to treat blood clots. Clopidogrel's ability to prevent platelet aggregation is completely owing to an active metabolite. When taking clopidogrel with other drugs that decrease CYP2C19 activity, such as esomeprazole, the conversion of clopidogrel to its active metabolite can be hampered.
  • Hypomagnesemia, both symptomatic and asymptomatic, has been recorded in individuals taking PPIs for at least three months, with the majority of instances occurring after a year.

Contraindication

Esomeprazole is contraindicated in patients with known hypersensitivity to substituted benzimidazoles or to any component of the formulation. Examples of benzimidazole derived drugs include-
