Fluconazole

Fluconazole, the first of a new subclass of synthetic triazole antifungal agents, is  available as tablets for oral administration, as a powder for oral suspension. Fluconazole is a highly selective inhibitor of lanosterol 14-α-demethylase, a fungus cytochrome P450 dependent enzyme. This enzyme is responsible for the conversion of lanosterol to ergosterol.

Fluconazole is indicated for the following infections-

  • Vaginal candidiasis
  •  Oropharyngeal and esophageal candidiasis
  • Cryptococcal meningitis

The dose for adult can vary for different infections. It is available as tablet, capsule, powder for suspension and IV infusion. The usual dose is 100-200 mg once per day. The maximum dose can be 400 mg daily for cryptococcal meningitis.

Fluconazole is well tolerated by most patients. The commonly reported side effects are-

  • Nausea
  • Vomiting
  • Diarrhea
  • Abdominal pain
  • Rash
  • Fluconazole should be administered with caution to patients with liver dysfunction.
  • Fluconazole should be administered with caution to patients with arrythmia.
  • Concomitant use of fluconazole and erythromycin has the potential to increase the risk of
    cardiotoxicity.
  • Fluconazole should be administered with caution to patients with renal dysfunction.

Contraindication

  • Fluconazole is contraindicated in patients who have shown hypersensitivity to fluconazole or to any of its excipients.
  • Coadministration of terfenadine is contraindicated in patients receiving fluconazole at multiple doses of 400 mg or higher.
  • Coadministration of other drugs known to prolong the QT interval and which are metabolized via the enzyme CYP3A4 such ascisapride, astemizole, pimozide, and quinidine are contraindicated in patients receiving fluconazole.

There is no known contraindication of fluconazole in terms of food and drinks.
