Haloperidol

Haloperidol is a first-generation antipsychotic that primarily blocks dopamine receptors (D2 receptors) in the brain. Its antipsychotic effects are thought to be related to this dopamine receptor blockade, particularly in the mesolimbic pathway. It also has antiemetic properties due to its action on the chemoreceptor trigger zone.

  • Haloperidol is indicated for the control of tics and vocal utterances of Tourette’s Disorder.

Injectable medication given intramuscularly in amounts ranging from 2 to 5 mg is employed for the swift management of acutely agitated schizophrenic individuals exhibiting symptoms of moderate to very severe intensity. Additional doses may be administered based on the patient's response, with the option of repeating them every hour, although intervals of 4 to 8 hours may also be deemed appropriate.

Common side effects associated with haloperidol include-

  • Extrapyramidal symptoms (e.g., tremor, rigidity, bradykinesia)
  • Drowsiness
  • Insomnia
  • Agitation
  • Headache
  • Extrapyramidal Symptoms (EPS):

    • Haloperidol may cause EPS, and the risk is higher at higher doses. Use the lowest effective dose to minimize this risk.
  • Neuroleptic Malignant Syndrome (NMS):

    • NMS, a potentially fatal condition, has been associated with antipsychotic use, including Haloperidol. Monitor for hyperthermia, muscle rigidity, altered mental status, and autonomic dysfunction.
  • Tardive Dyskinesia (TD):

    • Prolonged use of antipsychotics may lead to TD. Consider discontinuation if signs of TD appear.
  • Cardiovascular Effects:

    • Haloperidol may prolong the QT interval, leading to arrhythmias. Caution is advised in patients with cardiovascular disease.

Contraindication

Haloperidol is contraindicated in who are hypersensitive to this drug.

Haloperidol is contraindicated in severe toxic central nervous system depression or comatose states from any cause and in individuals who have Parkinson’s disease.
