Hydrochlorothiazide

Hydrochlorothiazide belongs to a group of drugs called thiazide diuretics. Hydrochlorothiazide causes diuresis by  blocking Naand Clion reabsorption from the distal tubule of the nephron.

Hydrochlorothiazide is indicated in the following health conditions-

  • Edema associated with congestive heart failure, hepatic cirrhosis, renal dysfunction and corticosteroid and estrogen therapy. 
  • Management of hypertension

Adults:

  • Edema: Adults should take 25 to 100 mg per day in a single or divided dose.
  • Management of hypertension: In adults, the usual starting dose is 25 mg per day, given as a single dose. The daily dose can be increased to 50 mg in one or two divided doses. Serum potassium levels are frequently reduced in doses more than 50 mg. An initial dose of 12.5 mg daily may be sufficient in certain patients (particularly the elderly).

Pediatrics:

  • The normal pediatric dosage is 1 to 2 mg/kg/day in single or two divided doses, not to exceed 37.5 mg per day in infants under the age of two and 100 mg per day in children aged two to twelve. Doses up to 3 mg/kg/day in two divided doses may be required in infants younger than 6 months.

  • Jaundice
  • Diarrhea
  • Vomiting
  • Renal problems
  • Anemia
  • Hyperglycemia
  • Hyperlipidemia
  • All patients receiving diuretic therapy should be observed for evidence of fluid or electrolyte imbalance: namely, hyponatremia, hypo-chloremic alkalosis, and hypokalemia.
  • When severe cirrhosis is present or after extended therapy, hypokalemia might develop, especially with rapid diuresis.
  • In hot weather, dilutional hyponatremia can arise in edematous individuals; the best treatment is water restriction rather than salt delivery, unless the hyponatremia is life threatening.
  • Certain patients using thiazides may experience hyperuricemia or develop acute gout.

Contraindication

Hydrochlorothiazide is contraindicated to the patients who are sensitive to hydrochlorothiazide or to other sulfonamide-derived drugs, such as-
