Ivermectin

Ivermectin is an anti-parasitic medication. Ivermectin binds to glutamate-gated chloride ion channels found in invertebrate nerve and muscle cells with high affinity, causing an increase in the permeability of cell membranes to chloride ions, hyperpolarization of the nerve or muscle cell, and, eventually, parasite death.

  • Strongyloidiasis of the intestine: Ivermectin is used to treat nondisseminated intestinal strongyloidiasis caused by the nematode parasite Strongyloides stercoralis. This indication is based on comparative and open-label clinical studies in which 64-100 percent of infected patients were cured after receiving a single 200-mcg/kg dosage of ivermectin.
  • Onchocerciasis: Ivermectin is used to treat onchocerciasis caused by the parasitic worm Onchocerca volvulus. This indication is based on 1427 patients in onchocerciasis-endemic areas of West Africa who participated in randomized, double-blind, placebo-controlled, and comparative trials. Diethylcarbamazine citrate was utilized in the comparative investigations (DEC-C).

Oral Onchocerciasis: 150 mcg/kg as a single dosage for cases caused by immature forms of Onchocerca volvulus; repeat treatment every 3-12 months until symptoms disappear.

If the child weighs less than 15 kg, the dose is the same as for adults.

Strongyloidiasis of the intestine:

Adults: a single dose of 200 mcg/kg. Alternatively, 200 mcg/kg daily for 1-2 days may be administered.

If the child weighs less than 15 kg, the dose is the same as for adults.

Topical/Cutaneous

Pediculosis of the head

As a 0.5 percent lotion for adults: Apply a generous amount (up to 1 tube) to thoroughly coat dry hair and scalp, then leave on for 10 minutes. Rinse the scalp and hair thoroughly with warm water, and remove nits with a fine-toothed comb as needed. This item is only to be used once.

6 months old child In the form of a 0.5 percent lotion: The dose is the same as for adults.

Topical/Cutaneous
Inflammatory lesions of rosacea

Adult: As 1% cream: Apply a thin layer onto the affected area(s) of the face once daily for up to 4 months; may repeat treatment course if needed. If there is no improvement after 3 months, discontinue treatment.
  • Mazzotti reaction (e.g. lymphadenitis, oedema, arthralgia, synovitis, tachycardia, fever, pruritus, urticaria)
  • Ophthalmic reaction particularly in patients undergoing treatment for onchocerciasis; transient aggravation of rosacea (topical cream).
  • Abdominal pain
  • Nausea
  • Vomiting
  • As a result of a reaction to the dying Demodex mites, patients may experience a transitory worsening of rosacea, which normally disappears within one week if the treatment is continued.
  • The treatment should be stopped if the condition worsens significantly with a strong cutaneous reaction.

Contraindication

Contraindicated in patients who are hypersensitive to any component of this product.
