Ketorolac Tromethamine

Ketorolac Tromethamine is a nonsteroidal anti-inflammatory drug (NSAID). Ketorolac Tromethamine works by inhibiting the synthesis of prostaglandins, which are substances that cause pain, inflammation, and fever. By blocking prostaglandin production, Ketorolac reduces pain and inflammation in the body.

 

  • Ketorolac Tromethamine is indicated for the short-term (up to 5 days) management of moderate to severe pain in adults.
  • It is commonly used after surgery, for the treatment of post-operative pain.
  • This drug should be reserved for short-term use when other pain management options are inadequate.
  • Patients age 17 to 64: 20 mg PO once followed by 10 mg q4-6 hours prn not >40 mg/day
  • Patients age > 65, renally impaired, and/or weight <50 kg (110 lbs): 10 mg PO once followed by 10 mg q4-6 hours prn not >40 mg/day 

 

The following side effects may occur with the use of ketorolac-

  • Nausea
  • Indigestion
  • Abdominal pain
  • Headache
  • Dizziness
  • Fatigue
  • Edema (swelling)
  • Bleeding complications (e.g., gastrointestinal bleeding)

 

  • Cardiovascular Risk: Ketorolac may increase the risk of cardiovascular events such as heart attack or stroke. It should be used with caution in patients with pre-existing cardiovascular conditions or risk factors.
  • Gastrointestinal Risk: Ketorolac can cause serious gastrointestinal side effects, including bleeding, ulceration, and perforation, which can be life-threatening, especially in older adults. Avoid its use in patients with a history of gastrointestinal bleeding or ulcers.
  • Renal Impairment: Ketorolac can cause acute renal failure or worsen pre-existing kidney problems. Use with caution in patients with impaired renal function.
  • Hypersensitivity: Patients with a known hypersensitivity to Ketorolac Tromethamine or other NSAIDs should not use this medication.

 

Contraindication

  • Ketorolac is contraindicated in patients with previously demonstrated hypersensitivity to ketorolac tromethamine.
  • The concomitant use of Ketorolac with probenecid & pentoxifylline is contraindicated.
  • Ketorolac is contraindicated in patients currently receiving aspirin or NSAIDs because of the cumulative risks of inducing serious NSAID-related adverse events.

 

 

 

  • Ketorolac is contraindicated in patients with active peptic ulcer disease, in patients with recent gastrointestinal bleeding or perforation and in patients with a history of peptic ulcer disease or gastrointestinal bleeding.
  • Ketorolac should not be given to patients who have experienced asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs. Severe, rarely fatal, anaphylactic-like reactions to NSAIDs have been reported in such patients
  • Ketorolac is contraindicated as prophylactic analgesic before any major surgery.  
  • Ketorolac is contraindicated for the treatment of peri-operative pain in the setting of coronary artery bypass graft (CABG) surgery 
  • Ketorolac is contraindicated in patients with advanced renal impairment or in patients at risk for renal failure due to volume depletion 
  • Ketorolac is contraindicated in labor and delivery because, through its prostaglandin synthesis inhibitory effect, it may adversely affect fetal circulation and inhibit uterine contractions, thus increasing the risk of uterine hemorrhage.
  • Ketorolac inhibits platelet function and is, therefore, contraindicated in patients with suspected or confirmed cerebrovascular bleeding, hemorrhagic diathesis, incomplete hemostasis and those at high risk of bleeding
