Azithromycin

Azithromycin is a broad spectrum macrolide antibiotic that kills bacteria by binding to the 50s subunit of its ribosome and inhibiting protein synthesis.

Azithromycin is indicated for the following infections caused by susceptible organisms-

  • Acute bacterial exacerbations of chronic obstructive pulmonary disease
  • Pharyngitis/tonsillitis
  • Uncomplicated skin and skin structure infections
  • Urethritis and cervicitis
  • Genital ulcer disease
  • Acute otitis media
  • Community-acquired pneumonia
  • Azithromycin is available as tablets, suspension and ophthalmic drops.
  • The usual dose of azithromycin for adults is- 500 mg once daily orally for 3 days or 500 mg once on day 1, then 250 mg once on days 2-5 for 4 days.
  • In individuals with sexually transmitted illnesses caused by Chlamydia trachomatis, a single dose of 1 gm or 500 mg once on day 1, followed by 250 mg once daily for the next two days may be given.

How to reconstitute Azithromycin suspension:

The most common treatment-related side effects in adult patients receiving multiple-dose regimens of azithromycin were related to the gastrointestinal system with diarrhea/loose stools, nausea and abdominal pain being the most frequently reported. 


 

  • Because azithromycin is primarily removed through the liver, it should be used with caution in patients who have compromised hepatic function.
  • Exacerbation of symptoms of myasthenia gravis and new onset of myasthenic syndrome have been reported in patients receiving azithromycin therapy.
  • Serious allergic reactions, including angioedema, anaphylaxis, and dermatologic reactions
    including Stevens Johnson Syndrome and toxic epidermal necrolysis have been reported rarely in
    patients on azithromycin therapy
  • Prolonged cardiac repolarization and QT interval, imparting a risk of developing cardiac
    arrhythmia and torsades de pointes, have been seen in treatment with macrolides, including
    azithromycin.

Contraindication

Azithromycin is contraindicated in patients with known hypersensitivity to any macrolide or ketolide such as-

Patients should also be cautioned not to take aluminum- and magnesium-containing antacids and azithromycin simultaneously. 

Azithromycin is contraindicated in patients with liver diseases.
