Pivmecillinam

Pivmecillinam is a narrow-spectrum penicillin antibacterial agent belonging to the aminopenicillin subgroup. Its mechanism of action involves inhibiting bacterial cell wall synthesis by binding to penicillin-binding proteins, which eventually leads to bacterial cell death.

 

  • Pivmecillinam is indicated for the treatment of uncomplicated urinary tract infections caused by susceptible strains of Escherichia coli and other Gram-negative bacteria.
  • It can also be used for the treatment of adults with acute uncomplicated cystitis due to mecillinam sensitive organisms.

 

The recommended dose for uncomplicated urinary tract infections in adults is 400 mg orally every 12 hours for 7 days.

 

  • The most common adverse reactions associated with pivmecillinam are gastrointestinal disturbances such as nausea, vomiting, and diarrhea.
  • Other adverse reactions may include hypersensitivity reactions such as skin rash, urticaria, and angioedema.

 

  • Pivmecillinam should be used with caution in patients with a history of hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
  • Patients with renal impairment may require dosage adjustment.
  • Pivmecillinam should be used with caution in patients with a history of gastrointestinal disease, particularly colitis.
  • Patients should be advised to report any signs of allergic reactions such as skin rash, hives, or difficulty breathing.
  • Prolonged use of pivmecillinam may result in the overgrowth of nonsusceptible organisms, including fungi, which may require treatment with antifungal agents.
