Propranolol Hydrochloride

Propranolol is indicated for the following health conditions-

  • Control of hypertension
  • Management of angina
  • Prophylaxis of migraine
  • Management of essential tremor
  • Management of anxiety
  • Adjunctive management of thyrotoxicosis
  • Prophylaxis of upper gastro-intestinal bleeding in patients with portal hypertension and esophageal varices

Hypertension:

  • The usual starting dose is 160mg per day, taken orally in the morning or evening. Most people have an acceptable reaction at this dosage. It can be increased in 80 mg increments if necessary until the appropriate response is obtained (up to a maximum of 320 mg daily).

Angina, essential tremor, thyrotoxicosis, and migraine prophylaxis:

  • The standard dose of 80mg per day, given orally in the morning or evening, may be sufficient for most people to achieve satisfactory control. The dose may be increased to 160mg per day, and subsequently to 240mg per day if necessary.

Anxiety:

  • 80 mg propranolol taken once a day should be enough to offer short-term relief from acute situational anxiety. Generalized anxiety, which requires longer-term treatment, usually responds well to the same dose. The dosage may be increased to 160 mg in some cases. After 6–12 months of treatment, patients should be reassessed. Continuation of treatment should be based on the patient's response.

 

Side effects commonly associated with Propranolol are-

  • Asthenia
  • Dizziness
  • Thrombocytopenia
  • Bradycardia
  • Hypotension
  • Dry eyes
 
  • Patients with a history of wheeze or asthma should only use propranolol if absolutely necessary.
  • In patients with ischemic heart disease treatment must not be discontinued abruptly. 
  • Patients having a history of severe anaphylactic reactions to a range of allergens may be more reactive to repeated challenges, whether accidental, diagnostic, or therapeutic, while taking beta blockers. The standard amounts of epinephrine used to treat allergic reactions may be ineffective in such patients.

Contraindication

Propranolon hydrochloride is contraindicated in patients with known hypersensitivity to propranolol hydrochloride or other beta antagonists, such as-
