Salmeterol

Salmeterol is a beta 2 adrenoceptor agonist. It is long acting bronchodilator aerosol. Salmeterol binds to the beta 2 adrenoceptor present is the trachea, bronchi & bronchioles and causes relaxation of the smooth muscle. This causes dilation of the airways.

Salmeterol is indicated in the following condition-

Asthma

Adults and adolescents 12 years and older: Two actuations of 25 micrograms salmeterol twice daily. In asthma patients with more severe airways obstruction up to four inhalations of 25 micrograms of salmeterol twice daily may be of benefit.

Children below twelve years of age: The safety and efficacy of Salmeterol Inhaler 25 micrograms have not been demonstrated in children. Therefore salmeterol Inhaler 25 micrograms is not recommended for use in children below twelve years of age.

COPD

Adults: Two actuations of 25 micrograms salmeterol twice daily.

Children: There is no relevant indication for use of salmeterol Inhaler 25 micrograms in children.

The following side effects were reported with the use of salmeterol-

  • Ear, nose, and throat Nasal/sinus congestion
  • Rhinitis
  • Headache
  • Bronchitis
  • Influenza

Data from a large placebo-controlled safety study that was stopped early suggest that salmeterol may be associated with rare serious asthma episodes or asthma-related deaths.

Use With Spacer or Other Devices: The safety and efficacy of Salmeterol Inhalation Aerosol when used with spacer or other devices have not been thoroughly investigated.

Cardiovascular and Other Effects: Inhaled salmeterol has no effect on the cardiovascular system when administered in recommended doses, but the cardiovascular and central nervous system effects seen with all sympathomimetic drugs (e.g., increased blood pressure, heart rate, excitement) can occur after use of salmeterol and may necessitate drug discontinuation.

Metabolic Effects:

When administered intravenously, doses of the related beta2-adrenoceptor agonist albuterol have been shown to aggravate preexisting diabetes mellitus and ketoacidosis. At the recommended doses, Salmeterol Inhalation Aerosol had no effect on glucose. Some patients may experience significant hypokalemia as a result of beta-adrenergic agonist medications, possibly due to intracellular shunting, which has the potential to cause adverse cardiovascular effects. The decrease is usually transient and does not necessitate supplementation.

  • Shake well before using.
  • The action of salmeterol Inhalation Aerosol may last up to 12 hours or longer. The recommended dosage (2 inhalations twice daily, morning and evening) should not be exceeded.
  • Inhalation of Salmeterol Aerosol is not intended to treat acute asthma or COPD symptoms, and additional doses should not be used for this purpose. Acute symptoms should be treated with a short-acting beta2-agonist, such as albuterol, inhaled (the physician should provide the patient with such medication and instruct the patient in how it should be used).
  • Patients should not discontinue Salmeterol therapy for asthma or COPD without consulting a physician or provider, as symptoms may recur after discontinuation.

Contraindication

Salmeterol is contraindicated in patients with a history of hypersensitivity to salmeterol or any other component of the drug product.
