Sertraline

Sertraline is an antidepressants from the class selective serotonin reuptake inhibitors. Sertraline potentiates serotonergic activity in the central nervous system through inhibition of neuronal reuptake of serotonin (5-HT).

Sertraline is a selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of:

  • Major depressive disorder (MDD)
  • Obsessive-compulsive disorder (OCD)
  • Panic disorder (PD)
  • Post-traumatic stress disorder (PTSD)
  • Social anxiety disorder (SAD)
  • Premenstrual dysphoric disorder (PMDD)

The recommended initial dosage and maximum sertraline dosage in patients with MDD, OCD, PD, PTSD, and SAD are shown in Table-

Indication Starting Dose  Therapeutic Range
Adults
MDD  50 mg 50-200 mg
OCD 50 mg
PD, PTSD, SAD 25 mg
Pediatric Patients
OCD (ages 6-12 years old)  25 mg 50-200 mg
OCD (ages 13-17 years old)  50 mg

Most common adverse reactions were

  • nausea
  • diarrhea
  • tremor
  • dyspepsia
  • decreased appetite
  • hyperhidrosis
  • ejaculation failure
  • decreased libido

  • Serotonin Syndrome: Risk increases when combined with other serotonergic agents (e.g., SSRI, SNRI, triptans), also when used alone. If this happens, stop taking Sertraline and start receiving supportive care.
  • Concurrent use of aspirin, nonsteroidal antiinflammatory drugs (NSAIDs), other antiplatelet drugs, warfarin, and other anticoagulants may increase the risk of bleeding.
  • Mania/Hypomania Activation: Screen patients for bipolar disorder.
In patients with seizure disorders, use with caution.
  • Angle Closure Glaucoma: Antidepressants, including Sertraline, should be avoided in patients with untreated anatomically narrow angles.

Contraindication

Sertraline is contraindicated in-

  • Known hypersensitivity to sertraline or excipients
  • Concomitant use of pimozide
  • Sertraline oral solution only: Concomitant use with
