Timolol Maleate

Timolol maleate eye drops, solution is a beta-adrenoreceptor blocking medication that is used topically to treat high intraocular pressure in a variety of diseases.

Timolol maleate is indicated in adults for-

  • Ocular hypertension
  • Chronic open-angle glaucoma
  • Secondary glaucoma
  • Recommended therapy is one drop of 2.5 mg/ml solution in the affected eye, once or twice (morning and evening) a day.
  • If clinical response is not adequate, dose may be changed to one drop of 5 mg/ml solution in each affected eye, once or twice (morning and evening) a day. If needed, Timolol maleate may be used with other agent(s) for lowering intra-ocular pressure. The use of two topical beta-adrenergic blocking agents is not recommended
  • Intra-ocular pressure should be reassessed approximately four weeks after starting treatment because response to timolol maleate may take a few weeks to stabilize.

Commonly associated side effects include-

  • Eye burning
  • Bradycardia
  • hypotension
  • arrhythmia
  • Raynaud's phenomenon
  • headache
  • dizziness
  • asthenia
  • abdominal discomfort
  • nausea
  • Caution should be exercised while treating patients with-
    • poor cardiac function
    • diabetes mellitus
    • myasthenia gravis
    • cerebrovascular insufficiency
    • atopy.
  • Avoid sudden withdrawal since it can worsen angina symptoms or cause a heart attack in people with coronary artery disease, or trigger a thyroid crisis in thyrotoxicosis patients.
  • Caution should be taken for those who are receiving significant surgery.
  • Hyperthyroidism and hypoglycemia symptoms may be masked. Angle-closure glaucoma should not be treated with ophthalmic solution alone.

Contraindication

Timolol is contraindicated in patients hypersensitive to timolol or any component of the product.

Timolol is contraindicated in patients with-

  • bronchial asthma
  • severe chronic obstructive pulmonary disease
  • sinus bradycardia
  • second or third degree atrioventricular block
  • overt cardiac failure
  • cardiogenic shock
