Candesartan Cilexetil

Candesartan belongs to a class of drugs called Angiotensin II receptor blockers or ARBs. It prevents Angiotensin II from binding to Angiotensin receptor 1 and hence antagonizing it's effects.

Candesartan is used to treat the following health conditions-

  • Hypertension
  • Heart Failure

Candesartan is available as tablets. It can be taken with or without meals.

Hypertension: Amias is taken once daily at an initial dose of 8 mg and thereafter at a maintenance dose of 8 mg. Within four weeks, the majority of the antihypertensive impact is achieved. For some people with uncontrolled blood pressure, the dose can be increased to 16 mg once daily, with a maximum of 32 mg once daily. The dosage of medication should be modified based on the patient's blood pressure response.

Heart Failure:

  • The usual recommended initial dose of Amias is 4 mg once daily. Up-titration to the target dose of 32 mg once daily (maximum dose) or the highest tolerated dose is done by doubling the dose at intervals of at least 2 weeks. 
  • Amias can be used in combination with other heart failure medications such as ACE inhibitors, beta-blockers, diuretics, and digitalis, or a combination of them. When mineralocorticoid receptor antagonists are poorly tolerated, amias can be used with an ACE inhibitor in patients with symptomatic heart failure while receiving adequate conventional heart failure medication.

Patients with renal impairment: In individuals with renal impairment, particularly those on hemodialysis, the initial dose is 4 mg. The dose should be adjusted based on the patient's response. In individuals with very severe or end-stage renal impairment (Creatinine clearance<15 ml/min), there is insufficient data.

Pediatrics: The recommended starting dose is 4 mg once daily.

The side effects associated with candesartan are-

  • Respiratory infection
  • Dizziness
  • Headache
  • Back pain
  • Angioedema
  • Renal problems
  • During the second and third trimesters of pregnancy, medications that act on the renin-angiotensin system such as candesartan, impair fetal renal function and increase fetal and newborn morbidity and death.
  • Candesartan for hypertension should not be given to children under the age of one year. Directly acting drugs on the renin-angiotensin system (RAS) can affect the development of immature kidneys.
  • In patients with renal artery stenosis, aortic or mitral valve stenosis, and obstructive hypertrophic cardiomyopathy, candesartan should be administered with caution.

Contraindication

Candesartan is contraindicated in the following health conditions-

  • Pregnancy
  • Severe hepatic problems (Cholestasis)
  • Children aged below 1 year
