Cefetamet Pivoxil Hydrochloride

Cefetamet Pivoxil Hydrochloride belongs to the class of antibiotics known as cephalosporins. Its mechanism of action involves inhibiting bacterial cell wall synthesis, leading to cell death. Cefetamet exerts its antibacterial effects by binding to and inhibiting the activity of penicillin-binding proteins, essential for the synthesis of the bacterial cell wall. This action results in the disruption of cell wall integrity and the eventual lysis of the bacteria.

Cefetamet Pivoxil Hydrochloride is indicated for the treatment of the following bacterial infections when caused by susceptible strains:

  1. Respiratory Tract Infections:

    • Upper respiratory tract infections
    • Lower respiratory tract infections
  2. Urinary Tract Infections: Uncomplicated urinary tract infections

  3. Skin and Skin Structure Infections: Uncomplicated skin and skin structure infections

Dosage may vary based on the type and severity of the infection, patient age, and renal function.

  1. Adults and Adolescents (12 years and older):

    • The usual dose for most infections is 250 mg to 500 mg every 12 hours.
    • Higher doses may be appropriate for certain infections.
  2. Pediatric Patients (6 months to 12 years):

    • The recommended dose is 7 mg/kg to 16 mg/kg every 12 hours, depending on the severity of the infection.
  3. Renal Impairment:

    • Dosage adjustment is necessary in patients with impaired renal function. Consult the prescribing information for specific dosing recommendations.

Adverse reactions are listed from most common to least common:

  • Gastrointestinal disturbances (nausea, vomiting, diarrhea)
  • Skin rashes
  • Allergic reactions (including itching and hives)
  • Headache
  • Eosinophilia
  • Transient increases in liver enzymes
  • Hematologic effects (including eosinophilia, thrombocytosis)
  • Hypersensitivity Reactions: Serious and occasionally fatal hypersensitivity reactions, including anaphylaxis, may occur. If a severe allergic reaction occurs, discontinue Cefetamet Pivoxil Hydrochloride and institute appropriate therapy.
  • Clostridioides difficile-associated Diarrhea (CDAD): CDAD has been reported with the use of nearly all antibacterial agents, including Cefetamet Pivoxil Hydrochloride. Evaluate if diarrhea occurs during or after treatment.
  • Renal Impairment: Adjust the dosage in patients with impaired renal function. Regular monitoring of renal function is advisable.
  • Coagulation Abnormalities: Cephalosporins, including Cefetamet Pivoxil Hydrochloride, may cause a disulfiram-like reaction with alcohol consumption. Avoid concurrent use.
  • Superinfection: Prolonged use may result in overgrowth of non-susceptible organisms. Careful observation of the patient is essential.

Contraindication

Cefetamet Pivoxil Hydrochloride is contraindicated in individuals with:

  1. Known hypersensitivity to cephalosporins or the components of the formulation.
  2. History of severe hypersensitivity reactions (anaphylaxis, Stevens-Johnson syndrome, etc.) to beta-lactam antibacterial drugs.
