Adefovir Dipivoxil

Adefovir is an acyclic nucleotide analog of adenosine monophosphate that is phosphorylated by cellular kinases to the active metabolite adefovir diphosphate. By competing with the natural substrate deoxyadenosine triphosphate and causing DNA chain termination after incorporation into viral DNA, adefovir diphosphate inhibits HBV DNA polymerase (reverse transcriptase).

Adefovir is indicated for the treatment of chronic hepatitis B in patients 12 years of age and older.

Adefovir 10 mg once daily, without regard to food, is the recommended dose in chronic hepatitis B patients 12 years of age and older with adequate renal function. The optimal treatment duration is unknown.

Most common adverse reactions were-

  • Asthenia
  • Headache
  • Abdominal Pain 
  • Nausea 
  • Flatulence
  • Diarrhea
  • Dyspepsia 
  • Patients who stop taking Adefovir may experience severe acute hepatitis exacerbations. In these patients, closely monitor hepatic function.
  • Chronic use of Adefovir may cause nephrotoxicity in patients at risk of or with underlying renal dysfunction. In these patients, keep a close eye on their renal function. A dose adjustment may be necessary.
  • In chronic hepatitis B patients with undiagnosed or untreated HIV infection, HIV resistance may develop.
  • The use of nucleoside analogues has been linked to lactic acidosis and severe hepatomegaly with steatosis, including fatal cases.
  • Adefovir dipivoxil in combination with lamivudine is used to treat patients with lamivudine-resistant HBV. Consider modifying treatment for all patients if serum HBV DNA remains above 1000 copies/mL despite continued treatment.

Contraindication

Adefovir contraindicated in patients with previously demonstrated hypersensitivity to any of the components of the product.
