Lovastatin
Lovastatin is a lipid lowering medication that acts by inhibiting the enzyme HMG Co-A reductase. HMG Co-A reductase is an enzyme essential for the synthesis of cholesterols.
Lovastatin is indicated for the following cases-
- Hyperlipidemia
- Prevention of Cardiovascular diseases (myocardial infarction, stroke, angina etc.)
- Coronary heart diseases
- Adult Patients: The usual recommended starting dose is 20 mg once a day given with the evening meal. The recommended dosing range of lovastatin is 10-80 mg/day in single or two divided doses; the maximum recommended dose is 80 mg/day. Doses should be individualized according to the recommended goal of therapy.
- Dosage in Patients taking Danazol, Diltiazem, or Verapamil: In patients taking danazol, diltiazem, or verapamil concomitantly with lovastatin, therapy should begin with 10 mg of lovastatin and should not exceed 20 mg/day
- Dosage in Patients taking Amiodarone: In patients taking amiodarone concomitantly with lovastatin, the dose should not exceed 40 mg/day.
- Adolescent Patients (10-17 years of age) with Heterozygous Familial Hypercholesterolemia: The recommended dosing range of lovastatin is 10-40 mg/day; the maximum recommended dose is 40 mg/day.
- Patients with renal impairment: In patients with severe renal insufficiency (creatinine clearance <30 mL/min), dosage increases above 20 mg/day should be carefully considered and, if deemed necessary, implemented cautiously.
Special precautions should be taken when implementing lovastatin in patients (with)-
- History of liver disease
- At risk of myopathy
- Alcoholism
- Inadequately controlled hypothyroidism
- Severe renal impairment
Contraindication
Lovastatin is contraindicated in patients hypersensitive to the medicine itself or other statins, such as-
