Lovastatin

Lovastatin is a lipid lowering medication that acts by inhibiting the enzyme HMG Co-A reductase. HMG Co-A reductase is an enzyme essential for the synthesis of cholesterols.

Lovastatin is indicated for the following cases-

  • Hyperlipidemia
  • Prevention of Cardiovascular diseases (myocardial infarction, stroke, angina etc.)
  • Coronary heart diseases
  • Adult Patients: The usual recommended starting dose is 20 mg once a day given with the evening meal. The recommended dosing range of lovastatin is 10-80 mg/day in single or two divided doses; the maximum recommended dose is 80 mg/day. Doses should be individualized according to the recommended goal of therapy.
  • Dosage in Patients taking Danazol, Diltiazem, or Verapamil: In patients taking danazol, diltiazem, or verapamil concomitantly with lovastatin, therapy should begin with 10 mg of lovastatin and should not exceed 20 mg/day
  • Dosage in Patients taking Amiodarone: In patients taking amiodarone concomitantly with lovastatin, the dose should not exceed 40 mg/day.
  • Adolescent Patients (10-17 years of age) with Heterozygous Familial Hypercholesterolemia: The recommended dosing range of lovastatin is 10-40 mg/day; the maximum recommended dose is 40 mg/day.
  • Patients with renal impairment: In patients with severe renal insufficiency (creatinine clearance <30 mL/min), dosage increases above 20 mg/day should be carefully considered and, if deemed necessary, implemented cautiously.

Side effects associated with lovastatin is usually mild and transient. The most commonly reported adverse reactions in patients treated with Lovastatin include:

  • arthralgia/ myalgia 
  • diarrhea
  • Headache
  • Dizziness
  • Weight gain
  • Rash
  • Blurred vision

Special precautions should be taken when implementing lovastatin in patients (with)-

  • History of liver disease
  • At risk of myopathy
  • Alcoholism
  • Inadequately controlled hypothyroidism
  • Severe renal impairment

Lovastatin is contraindicated in patients hypersensitive to the medicine itself or other statins, such as-
