Norepinephrine

Norepinephrine is a hormonal medication used to treat severe allergic reactions (anaphylaxis), as well as to treat certain types of heart failure. It works by narrowing blood vessels and opening airways in the lungs, making it easier to breathe and increasing blood flow to the heart. It also increases the strength of heart contractions and the rate at which the heart beats.

Norepinephrine is indicated for the following conditions-

  • Treatment of anaphylaxis (severe allergic reactions)
  • Treatment of cardiogenic shock
  • To improve the survival rate after cardiac arrest
  • To increase the heart rate in certain types of heart failure.

The recommended dosage of norepinephrine are as follows-

  • Anaphylaxis: 0.3 to 0.5 mg given subcutaneously or intramuscularly.
  • Cardiogenic shock: 2 to 10 mcg/min by intravenous infusion.
  • Heart failure: 2 to 10 mcg/min by intravenous infusion.

  • Common side effects include- nervousness, headache, sweating, palpitations, and nausea.
  • Less common side effects include- allergic reactions, chest pain, and high blood pressure.
  • Rare side effects include- seizures, heart attack, and stroke.
  • Undiluted ophthalmic administration is associated with corneal endothelial damage.
  • Do not inject into buttocks, digits, hands, or feet.
  • May aggravate angina pectoris or produce ventricular arrhythmias, particularly in patients with underlying heart disease, administer with caution when used intramuscularly or subcutaneously.
  • Patients with hyperthyroidism, Parkinson’s disease, diabetes, and pheochromocytoma are at greater risk of having adverse reactions when used intramuscularly or subcutaneously.
  • Presence of sulfite in this product should not deter use for anaphylaxis.

Contraindication

Norepinephrine is contraindicated in patients with a history of hypersensitivity to norepinephrine or any of its excipients.

  • Norepinephrine is contraindicated in patients with hypertension or severe coronary artery disease.
  • Norepinephrine is contraindicated in patients with pheochromocytoma (a rare tumor of the adrenal gland).
