Aliskiren + Hydrochlorothiazide
Aliskiren + Hydrochlorothiazide is a combination of renin inhibitor and thiazide diuretic. Aliskiren inhibits the enzyme renin, thereby reducing the conversion of angiotensinogen to angiotensin I. Hydrochlorothiazide is a thiazide diuretic that inhibits sodium reabsorption in the distal convoluted tubules of the kidney, promoting diuresis and reducing blood volume.
- Aliskiren + Hydrochlorothiazide is indicated for the treatment of hypertension (high blood pressure) in patients who require combination therapy.
- It may be used as initial therapy in patients likely to need multiple drugs to achieve blood pressure goals.Initial dose: 150 mg-12.5 mg or 150 mg-25 mg once daily
Maximum dose: Aliskiren-hydrochlorothiazide 300/25 mg orally daily
- The usual adult dosage for aliskiren + hydrochlorothiazide is 150 mg-12.5 mg or 150 mg-25 mg once daily
- Maximum dose: Aliskiren + hydrochlorothiazide 300/25 mg orally daily
Adverse reactions associated with Aliskiren + Hydrochlorothiazide include-
- Hypotension (low blood pressure)
- Hyperkalemia (high potassium levels)
- Dizziness
- Headache
- Fatigue
- Diarrhea
- Cough
- Nausea
- Abdominal pain
- Nasopharyngitis (inflammation of the nose and throat)
- Hypotension: Aliskiren + Hydrochlorothiazide can cause symptomatic hypotension, particularly in patients with volume depletion.
- Hyperkalemia: Monitoring of potassium levels is recommended, especially in patients with renal impairment or diabetes.
- Renal impairment: Aliskiren + Hydrochlorothiazide is not recommended in patients with severe renal impairment or on dialysis.
- Fetal toxicity: Aliskiren can cause fetal harm when administered to pregnant women. It should be discontinued as soon as pregnancy is detected.
- Electrolyte abnormalities: Regular monitoring of electrolytes, including sodium, potassium, and magnesium, is advised.
Contraindication
Aliskiren + Hydrochlorothiazide is contraindicated in-
- patient with history of hypersensitivity to any component of the medication
- Concomitant use of angiotensin converting enzyme (ACE) inhibitors or angiotensin receptor blockers (ARBs) in patients with diabetes
