Gemifloxacin

Gemifloxacin is an antibiotic of the fluoroquinolone class. It kills bacteria by inhibiting DNA synthesis. 

Gemifloxacin is indicated for-

  • Exacerbation of chronic bronchitis caused by Streptococcus pneumoniae, Haemophilus influenzae, Haemophilus parainfluenzae, or Moraxella catarrhalis.
  • Community acquired pneumonia caused by Streptococcus pneumoniae (including multi-drug resistant strains), Haemophilus influenzae, Moraxella catarrhalis, Mycoplasma pneumoniae, Chlamydia pneumoniae, or Klebsiella pneumoniae.
  • Gemifloxacin is available as tablets.
  • The usual dose for gemifloxacin 320 mg once daily for 5 to 7 days. The dosage may vary depending upon the patient's condition and severity of the infection.
  • It is not necessary to alter the dose in patients with a creatinine clearance of more than 40 mL/min. Patients with a creatinine clearance of less than 40 mL/min should have their dosage adjusted.
  • Patients who need hemodialysis or continuous ambulatory peritoneal dialysis (CAPD) on a regular basis should take 160 mg every 24 hours.

The common side effects caused by gemifloxacin include-

  • Nausea and other gastrointestinal disturbances
  • Headache & Dizziness
  • Joint pain
  • Skin rashes
  • Long QT syndrome
  • It can also halt bone and cartilage growth.
  • Clostridium difficile associated diarrhea.
  • Fluoroquinolones, including gemifloxacin, are associated with an increased risk of tendinitis and tendon rupture in all ages. This adverse reaction most frequently involves the Achilles tendon, and rupture of the Achilles tendon may require surgical repair.
  • Patients using gemifloxacin should be encouraged to drink plenty of water. 
  • In individuals with suspected or established CNS problems, such as epilepsy, or other variables that predispose to seizures and convulsions, it should be administered with caution. 
  • Avoid in patients with known QT prolongation, hypokalemia.

Contraindication

Gemifloxacin is contraindicated in patients with known hypersensitivity to Gemifloxacin and other quinolones such as-

Gemifloxacin should not be taken with milk, mineral supplements or antacids.

 Gemifloxacin is contraindicated in-

  • Pregnancy & Lactation
  • Children under 18
  • Tendonitis
